白内障手術後の眼圧

Intraocular pressure following cataract surgery

　白内障術後の99眼について，術後長期の眼圧の推移とC値の変化を検討した。

　非緑内障眼の眼圧は，偽水晶体眼では術後低下する傾向にあった。無水晶体眼では，初期には眼圧がやや低下し，術後長期間の経過ではむしろ徐々に上昇する傾向を認めた。

　C値は，眼内レンズ（IOL）嚢内固定眼で術後改善したが，無水晶体眼では悪化した。

　緑内障眼では，濾過手術をしない白内障IOL単独手術の症例でも，術後眼圧が良好に保たれ，術後C値も改善を認めた。

　以上のことから，緑内障白内障合併眼には，白内障手術としてIOLの嚢内固定を第一とし，眼圧コントロールが不良な症例に対してのみ緑内障手術を追加すればよいと結論された。

We evaluated the intraocular pressure (IOP) and coefficient of outflow in 99 eyes after cataract surgery. The IOP value tended to decrease slightly in pseudophakic eyes, particularly in those with intracapsular fixation. IOP in aphakic eyes showed gradual rise after initial decrease after surgery. The facility of outflow improved in eyes with pseudophakic eyes with intracapsular fixation and decreased in aphakic eyes. The IOP and facility of outflow in glaucomatous eyes tended to remain stationary after cataract surgery. The findings suggest that intraocular lens implantation with intracapsular fixation is the first choice in eyes with cataract and glaucoma.
