脈絡膜新生血管膜摘出術における疾患別の手術成績

Results of surgical removal of choroidal neovascular membrane of different etiologies

　新生血管黄斑症に対し中心窩下脈絡膜新生血管膜摘出術を行った症例について，術前視機能，視力低下から手術までの期間，新生血管膜径と，術後視機能を疾患別に検討した。対象は加齢黄斑変性43例43眼，特発性脈絡膜新生血管7例7眼，網膜色素線条症7例7眼である。3疾患の間で術前視機能，視力低下から手術までの期間，新生血管膜径に有意差はなかった。術後6か月，12か月における視力，ハンフリー視野検査での成績にも有意差はなかった。また，各疾患とも術後6か月から術後12か月で視力は改善傾向にあり，新生血管膜摘出術は有効であると考えた。

We reviewed the outcome of surgical removal of choroidal neovascular mebranes (CNMs) in 57 eyes of 57 patients. The series comprised age-related CNV 43 eyes, idiopathic CNV 7 eyes and angioid streaks 7 eyes. There were no differences among the 3 groups regarding preoperative visual acuity, duration between onset of visual failure and surgery, and the size of neovascular membrane. There were no differences among the 3 groups regarding postoperative visual acuity at 6 and 12 months, and central visual field as estimated by automated perimeter of Humphrey. There was a general tendency in each of the 3 groups for the visual acuity to improve between 6 and 12 postoperative months. The findings show the effectiveness of surgical removal of CNVs.
