（17-D-1）　69歳女性が両眼の網膜色素線条と脈絡膜血管新生で受診した。右眼にはレーザー光凝固を行い，その8か月後に新生血管膜を摘出した。最終視力は0.6であった。左眼には手術による新生血管膜摘出手術のみを行い，視力は0.2から0.4に回復したが，再発のために最終視力は0.08であった。摘出組織を免疫化学的に検索した。血管内皮増殖因子（vascular endothelial growth factor：VEGF）はレーザーを行わなかった左眼でより強く現れていた。レーザー光凝固がVEGFの発現を抑制し，手術にも好影響を与えることが推測された。

A 69-year-old female presented with angioid streaks complicated by choroidal neovascular membrane (CNVM) in both eyes. Her right eye was treated by laser photocoagulation and, 8 months later, by surgical removal of CNVM. The visual acuity improved to 0.6. Her left eye underwent surgery alone followed by improvement of visual acuity from 0.2 to 0.4. Recurrence of CNVM resulted in the final visual acuity of 0.08 in the left eye. Immunohistological studies of excised CNVM showed more intense immunoreactivity for vascular endothelial growth factor (VEGF) in the left eye. The findings suggest that laser photocoagulation may suppress the expression of VEGF in CNVM and may favorably affect the surgical outcome.
