要約 目的:ブリモニジン点眼後の緑内障眼の視神経乳頭の血流変化の報告。対象と方法:他の薬物による治療が困難な,広義の原発開放隅角緑内障11例11眼を対象とした。1か月間の休薬期間を置いたのち,0.1%ブリモニジン酒石酸塩点眼薬の1日2回点眼を開始した。点眼開始前と,直後,1か月後,3か月後に視神経乳頭血流を測定した。測定には,レーザースペックルフローグラフィを用い,mean blur rate(MBR)として評価した。結果:眼圧は点眼前と比較し,1か月後と3か月後に有意に下降した。乳頭の血流量は,点眼前と比較し,1か月後と3か月後では差がなかった。全身血圧,脈拍,眼灌流圧は,点眼前と比較し,1か月後と3か月後では差がなかった。結論:緑内障眼に対する3か月間のブリモニジン点眼で,眼圧は有意に下降し,乳頭の血流には変化がなかった。
Abstract. Purpose:To report changes in microcirculation in the optic nerve head following an instillation of 0.1% brimonidine ophthalmic solution in eyes with primary open-angle glaucoma. Cases and Method:This study was made on 11 eyes of 11 patients of primary open-angle glaucoma. After one month of washout period, they started instillation of brimonidine ophthalmic solution twice daily. Microcirculation in the optic nerve head was measured using laser speckle flowgraphy with mean blur rate serving as parameter. Results:Intraocular pressure decreased significantly after one and 3 months of treatment. There was no significant change in microcirculation in the optic nerve head up to 3 months of treatment. There was no significant change in systemic blood pressure, pulse rate or ocular perfusion pressure up to 3 months of treatment. Conclusion:Instillation of 0.1% brimonidine ophthalmic solution was followed by decrease in intraocular pressure and no change in microcirculation in the optic nerve head up to 3 months of follow-up.
