リパスジル塩酸塩・ブリモニジン酒石酸塩配合点眼液の眼圧下降効果の検討

要約　目的：緑内障に対する新規配合点眼薬であるリパスジル塩酸塩・ブリモニジン酒石酸塩配合点眼液（ROCK＋α）の眼圧下降効果を検討し，他の配合点眼液との比較を行う。

対象と方法：2022年12月〜2024年5月に大街道もり眼科医院でROCK＋αを処方された30例30眼，対照群としてドルゾラミド塩酸塩・チモロールマレイン酸塩配合点眼液（β＋CAI）を投与された8例8眼，ブリモニジン酒石酸塩・ブリンゾラミド配合点眼液（α＋CAI）を投与された患者15例15眼を対象とした。投与開始後1か月，3か月，6か月の眼圧をベースラインと比較し，各時点での眼圧下降率について対照群を比較した。処方前後の成分数が変わらない症例を除外した群を強化例として別個に検討した。

結果：ROCK＋α投与群全体では，3か月（p＝0.016），6か月（p＝0.01）でベースラインからの下降を認めた。強化例では1か月（p＝0.014），3か月（p＝0.027），6か月（p＝0.001）のいずれにおいても下降を認めた。対照群との比較では3か月において全体でROCK＋α：10.9±11.9％，α＋CAI：1.2±9.0％（p＝0.0075），強化例でROCK＋α：13.7±12.9％，α＋CAI：3.7±8.3％（p＝0.014）と，α＋CAIよりROCK＋αで高い眼圧下降を認めた。

結論：リパスジル塩酸塩・ブリモニジン酒石酸塩配合点眼液は既存の配合点眼液より優れた眼圧下降効果を示した。

Abstract　Purpose：This study evaluated the intraocular pressure（IOP）-lowering efficacy of brimonidine/ripasudil fixed-combination eye drops（ROCK＋α）compared with that of other combination eye drops.

Subjects and Methods：Thirty eyes of 30 patients prescribed ROCK＋α between December 2022 and May 2024 were analyzed. Control groups included 8 eyes treated with dorzolamide/timolol（β＋CAI）and 15 eyes treated with brimonidine/brinzolamide（α＋CAI）. IOP was measured at baseline and after 1, 3, and 6 months. Patients with unchanged active ingredients were excluded from the main analysis and were assessed as a subgroup.

Results：Significant IOP reductions were observed in the ROCK＋α group at 3（p＝0.016）and 6（p＝0.01）months. In the subgroup, the reductions were significant at 1（p＝0.014）, 3（p＝0.027）, and 6（p＝0.001）months. Compared to α＋CAI, ROCK＋α showed greater reductions at 3 months（overall：10.9±11.9％ vs. 1.2±9.0％, p＝0.0075；subgroup：13.7±12.9％ vs. 3.7±8.3％, p＝0.014）.

Conclusions：Brimonidine/ripasudil fixed-combination eye drops demonstrated superior IOP-lowering effects compared to existing combination eye drops.
