Abstract. Purpose:To report changes in microcirculation in the optic nerve head following instillation of tafluprost or latanoprost. Cases and Method:A randomized comparative study was made on 24 patients with primary open-angle glaucoma. Eleven patients received instillation of 0.005% tafluprost and 13 patients received latanoprost. They were followed up for 6 months. Laser speckle flowgraphy was used to quantitate microcirculation in the optic nerve head with mean blur rate serving as the indicator. Results:Intraocular pressure in both groups decreased significantly after instillation for one month or longer. There was no change in systemic blood pressure, pulse rate or perfusion pressure. In eyes receiving tafluprost, blood circulation increased in the optic nerve head except in the nasal sector. There was no change in optic nerve head circulation in eyes receiving latanoprost. Conclusion:Instillation of tafluprost may increase microcirculation in the optic nerve head in eyes with primary open-angle glaucoma
