To assess effects of dobutamine in infarcted myo-cardium, 10 patients with old anterior myocardial infarction were underwent hemodynamic and myo-cardial metabolic analyses. All patients were recei-ved intravenously 5 μg/kg/min and 10 μg/kg/min of dobutamine in order. Dobutamine (5 μg/kg/min vs 10 μg/kg/min) increased heart rate (+4%, +37%), cardiac index (+23%, +52%), mean aortic pressure (+12%, +13%), great cardiac vein flow (+21%, +92%) and coronary sinus flow (+25%, +85%) and decreased diastolic pulmonary artery pressure, but increased regional myocardial oxygen consumption (+19%, +63%) and deteriorated myocardial lactate extraction ratio (from 31.2% to 21.5%, 10. 6%). External mechanical efficacy increased (+21%, +8%) respectively. We conclude that dobutamine has ino-tropic action and increase coronary blood flow in ischemic regions. According to the external mecha-nical efficacy, low doses dobutamine (5 μg/kg/min) is preferable to ischemic heart although it reveals anerobic state in part.
