要旨

近年AIはさまざまな分野にて活用され，特にAIと親和性の高い画像診断での活用が注目を集めている。食道，胃でのAIによる病変の検出，腫瘍の鑑別が報告され，AI単体での診断能だけではなく，AIと医師との比較やAIによる上乗せ効果の検証も行われている。また，最近では，生成AIを活用した内視鏡検査の自動レポーティング作成や内視鏡検査のクオリティを客観的に判断するAI（CADq）の開発も盛んになってきており，画像診断AIと同様に高い精度が示されている。すでに臨床使用されている製品もあることから，今後は前向き研究を重ねて，実臨床でのAIの有用性を明らかにする必要がある。消化器内視鏡診断・治療は本邦が世界トップレベルの医療水準を保っており，内視鏡AIも日本から世界に通用する産業になることが期待される。本稿では，各種内視鏡AIの現状と今後の展望について概説する。

　Recently, artificial intelligence (AI) has been utilized in various fields including medicine, especially in the realm of diagnostic medical imaging. Several AI systems supporting the diagnosis of upper gastrointestinal cancer in endoscopic images have been developed, and previous studies showed their usefulness. Other than AI systems for gastrointestinal cancers, AI systems to support quality control of endoscopy or to automatically generate endoscopic reports have also been developed. Some of these AI systems are already used in real clinical practice, and their performance in the real world will be demonstrated in the near future. Gastrointestinal endoscopies originated in Japan, and Japan is expected to be the center of endoscopic AI as well. In this article, we outline the current status and future prospects of endoscopic AI.
