内視鏡AI支援が有用であった胃癌症例の1例

A Case of Gastric Cancer Effectively Detected with AI-assisted Endoscopy

要旨●自己免疫性胃炎（AIG）は，胃体部にびまん性の萎縮を呈し，神経内分泌腫瘍や胃癌の合併が知られている．AIGでは高度な萎縮を背景に病変の視認性が低下し，病変の発見が困難となることがある．本症例は，AIGを背景に発生した未分化型進行胃癌であり，サーベイランス内視鏡中に病変は正面から観察・記録されていたにもかかわらず，当初は見逃されていた．後日，内視鏡AI“gastroAI^TM model-G2”による後解析により当該病変が検出された．視認困難な病変に対するAIの有効性が確認され，診断精度の向上や見逃し防止，今後のリアルタイム診断支援への応用可能性が示唆された．

　Autoimmune gastritis（AIG）, characterized by diffuse atrophy of the gastric corpus, is associated with gastric neuroendocrine tumors and gastric cancer. In AIG, severe mucosal atrophy can reduce visibility, making the detection of lesions difficult. We report a case of poorly differentiated advanced gastric cancer arising in the background of AIG. Although the lesion was recorded and observed frontally by a surveillance endoscopy, it was initially overlooked. A retrospective analysis using the AI-assisted endoscopy system “gastroAI^TM model-G2” successfully identified the lesion. Our case highlights the utility of AI in improving diagnostic accuracy and preventing missed lesions, suggesting the potential application of AI in real-time clinical practice.
