消化管内視鏡AIの開発と現状―下部消化管―

Development and present status of artificial intelligence (AI) for lower gastrointestinal (GI) endoscopy

要旨

人工知能（AI）技術の発展により，AIによる画像診断支援の研究開発が盛んである。特に消化器内視鏡領域においては，国内外問わず多くのAIを用いた研究が報告されている。内視鏡診療におけるAIの役割は，病変の検出支援と鑑別支援に大別される。いずれにおいても，すでに一部の製品が規制をクリアし市販されており，実臨床における有用性が報告されている。本稿では第一にわれわれが開発に携わったEndoBRAIN^Ⓡの開発経緯に関してまとめ，薬機法承認を得るまでの課題などを共有する。第二にこれまで報告されている大腸内視鏡AIの臨床研究をレビューする。

　With the advancement of artificial intelligence (AI) technology, research and development for AI-assisted image diagnosis is emerging. Especially in the field of gastrointestinal endoscopy, numerous studies employing AI have been reported domestically and internationally. The roles of AI in endoscopic diagnosis can be broadly classified into lesion detection support and differential diagnosis support. Already, some products have passed regulations and have been marketed, with their clinical usefulness being reported. In this paper, firstly, we summarize the development process of EndoBRAIN^Ⓡ, which we have been involved in, sharing the challenges faced up until obtaining approval under the Pharmaceuticals and Medical Devices Act. Secondly, we review the clinical studies of colonoscopy AI that have been reported so far.
