乳癌術後の経過観察：画像検査の適応とポイントを中心に

乳癌の初期治療は病期およびサブタイプにより決定される。初期治療には手術・放射線療法といった局所治療と，ホルモン療法，化学療法や分子標的治療などの全身治療が含まれる。初期治療後は，定期診察および画像検査などで局所再発，遠隔転移のサーベイランスを行うのが一般的である。本稿では各種ガイドラインをふまえ，乳癌術後の経過観察における画像検査の適応とポイントを中心に概説する。

Initial treatment plan for breast cancer is determined by clinical staging and subtype of the lesion. Following this initial treatment, it is common to perform surveillance for local recurrence and distant metastasis by periodical physical examinations and proper imaging studies. There is a growing trend for choosing optimal imaging modality and interval of postoperative surveillance. This choice will be based not only on the evidence on guidelines but also will take into account the individual patient’s background and risk of recurrence and metastasis. Since the imaging findings of breast cancer recurrence and metastasis are often nonspecific, it is important to keep in mind the individual patient’s information when reading the images and to appropriately interpret the findings. These processes will lead to proper patient management.
