肺大細胞神経内分泌癌がSFT（solitary fibrous tumor）に転移したtumor-to-tumor metastasisの1例

A case of tumor-to-tumor metastasis from large cell carcinoma of the lung to solitary fibrous tumor

ある腫瘍に他の悪性腫瘍の転移が生じるいわゆる “tumor-to-tumor metastasis” はまれな病態ではあるが，古くから報告が行われている。レシピエントとなる腫瘍としては悪性では腎癌，良性では髄膜腫の報告が多く，孤立性線維性腫瘍（solitary fibrous tumor：SFT）がレシピエントとなる症例は非常にまれである。また，tumor-to-tumor metastasisは非常にまれなため術前診断が困難であり，多くの症例が生検や切除後に診断されている。今回我々は肺大細胞神経内分泌癌がSFTに転移し，¹⁸F-FDG PET/CTの経過から，tumor-to-tumor metastasisを鑑別として考えられた1例を経験したため，若干の文献的考察を加えて報告する。

Tumor to tumor metastasis is a rare but well documented phenomenon. Solitary Fibrous Tumor（SFT）is rare as a recipient of this phenomenon and little cases are reported previously. Because this phenomenon is rare, the preoperative diagnosis is difficult and it is often proved by an operation or autopsy. We present a case of 60’s years old man with history of lung cancer and gingival cancer who presented with a SFT of the supraclavicular fossa containing a metastatic large cell neuroendocrine carcinoma. In this case, the progress of ¹⁸F-FDG PET/CT findings contributed to the preoperative differential diagnosis.
