肺MALTリンパ腫からびまん性大細胞型B細胞リンパ腫への形質転換における画像的変化を観察できた1例

A case of pulmonary mucosa-associated lymphoid tissue lymphoma transforming into diffuse large B-cell lymphoma

粘膜関連リンパ組織を起源とする低悪性度B細胞由来リンパ腫である節外性濾胞辺縁帯リンパ腫（extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue：以下，MALTリンパ腫）は，時にびまん性大細胞型B細胞リンパ腫（diffuse large B-cell lymphoma：DLBCL）へ形質転換することが知られているが，画像所見に関する報告はほとんどない。今回，肺MALTリンパ腫からDLBCLへの形質転換を画像的に経過観察することができた1例を報告する。

We report a case of pulmonary extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue（MALT lymphoma）transforming into diffuse large B-cell lymphoma（DLBCL）. A woman in her seventies was detected a consolidation with a sharp margin in the right middle lung lobe on CT. Air bronchogram and CT angiogram sign were seen in the lesion. MALT lymphoma was suspected by transbronchial lung biopsy. Twenty-five months later, the lung lesion increased in size and revealed an expansive heterogeneous mass on CT, and the air bronchogram and CT angiogram sign in the lesion became inconspicuous. DLBCL was confirmed by CT-guided biopsy.
