気管支腺腫/線毛性粘液結節性乳頭状腫瘍の1例

A case of bronchiolar adenoma ciliated muconodular papillary tumor

線毛性粘液結節性乳頭状腫瘍（ciliated muconodular papillary tumor：CMPT）は2002年に石川^1）によって提唱された肺末梢に発生するまれな腫瘍である。偶発的に発見されることが多いが，画像所見からは悪性腫瘍との鑑別が困難であるため，その診断には病理組織検査が重要となっている。低線量CT肺癌スクリーニングが普及するにつれて，報告が増えており注目されている疾患である。今回我々はCMPTの1例を経験したため文献的考察を加えて報告する。

A woman in her 70 s was referred to our hospital because of a lung nodule increasing in size during 2 years follow-up. Chest CT showed a11-mm dense ground glass nodule with air-bronchogram in the left lower lobe. FDG-PET showed slightly increased FDG uptake in the nodule（SUVmax 1.88）. Wedge resection by video-assisted thoracic surgery was performed, and a specimen showed papillary proliferation with a mixture of ciliated, mucinous and basaloid cells. The findings indicated ciliated muconodular papillary tumor（CMPT）. CT findings of CMPT are similar to lung adenocarcinoma. Distinguishing between them by imaging is difficult and histological diagnosis is important.
