孤立性線維性腫瘍（solitary fibrous tumor：SFT）は全身の間葉系組織に発生するまれな腫瘍で，線維芽細胞あるいは筋線維芽細胞から発生すると考えられている。頭蓋内では主として硬膜に発生し，かつて血管周皮腫（hemangiopericytoma：HPC）と呼ばれていた腫瘍型も含むため，WHO分類第5版（2016年）ではSFT/HPCと併記されていたが，いずれもNAB2-STAT6の融合遺伝子をもつことからWHO分類第6版（2021年）ではSFTへと統一されている。SFTは転移するまでの期間は高悪性度のものほど短いが，10年以上という長い期間が経過しても転移することが知られている。

A male patient in his forties was incidentally diagnosed with an intracranial solitary fibrous tumor （SFT）following the onset of right homonymous hemianopia. Seven years after initial tumor resection, chest radiography performed during hospitalization for local recurrence demonstrated multiple small nodular opacities in the right pulmonary hilum and bilateral lung fields. The patient was subsequently referred to our department for CT-guided lung biopsy, which confirmed pulmonary and lymph node metastases of SFT. Given the aggressive biological behavior of high-grade intracranial SFTs, long-term systemic surveillance is warranted, and multidisciplinary collaboration with neurosurgeons is of paramount importance.
