腎癌の全経過において肝転移がみられるのはまれではないが，初診時に同時性肝転移がみられるのは比較的珍しい。さらにその肝転移が嚢胞状であった場合，転移と正しく診断するのは難しいことがありうる。今回我々は，腎癌の同時性嚢胞性多発肝転移の1例を経験した。嚢胞性肝転移の経過がCTで追えた症例でもあり，文献的考察を含めて報告する。

A septuagenarian female having past history of left RCC visited our institute complaining abdominal distension derived from enlargement of multiple hepatic cysts. They had been simultaneously existed on initial CT before nephrectomy but became symptomatic as their enlargement and intracystic hemorrhage. CT and MRI showed they did not have mural nodules nor peripheral enhancing rims. Right hepatectomy was performed under clinical diagnosis of polycystic liver disease. Histopathological examination revealed cystic liver metastases of RCC.
