気管支原性嚢胞は胎生期に迷入した前腸由来の呼吸器原基から発生する嚢胞性腫瘤である。大部分は縦隔や肺内に発生するが，まれに頸部や腹部，脊柱管内などに発生することも知られている。今回，左後腹膜に発生した気管支原性嚢胞の1例を経験したため，文献的考察を加えて報告する。

A left retroperitoneal cyst was found on screening ultrasonography in a 32-year-old healthy woman. Computed tomography showed a hyperdense cystic mass with a thin and calcified wall adjacent to the left diaphragmatic crus. Magnetic resonance imaging showed hyperintensity both on T2-weighted images and fat-saturated T1-weighted images. The preoperative diagnosis of retroperitoneal bronchogenic cyst was confirmed intraoperatively and histopathologically. We present an embryological hypothesis of the relationship between the left diaphragmatic crus and this rare disease.
