肝嚢胞内に発生したchronic expanding hematomaと考えられた1例

A case of chronic expanding hematoma arising in the hepatic cyst

肝嚢胞に出血した場合，感染や出血傾向などがなければ，経過で吸収されるのが一般的である。ただ，まれに造影効果を伴う壁在結節を有する出血性肝嚢胞がみられ，粘液性嚢胞性腫瘍との鑑別が困難と報告されている。今回，長期の経過が追跡できた壁在結節を有する出血性肝嚢胞の症例を経験したので，その画像所見および病理学的所見をふまえて報告する。

We have experienced hemorrhagic hepatic cyst with mural nodule occurring and increasing over time. In the MRI T2 weighted image, the mural nodule in the hepatic cyst had a high intensity inside and a low intensity rim. Dynamic studies demonstrated focal enhancement during the early phase coupled with progressive centrifugal enhancement during the delayed phase in the mural nodules. This mural nodule was pathologically thought to be chronic expanding hematoma generated in the hepatic cyst.
