　腹腔鏡下胆嚢摘出術は広く普及して胆嚢結石症に対する標準術式となったが，内視鏡下手術はまだ歴史が浅いこともあって，腹腔鏡下胆嚢摘出術以外の手術の普及率はまだまだ高いとはいえない．これらの手術を広く普及させるためには従来からのトレーニング法だけでなく，進んだ通信技術による遠隔医療や大学病院・センター病院などの大病院と地域の関連病院などの中小病院を結ぶネットワークを利用して，各学会のガイドラインにしたがって研修中の医師を教育するだけでなく，指導する立場にある専門家の育成を進めることが重要であると考える．

Laparoscopic cholecystectomy has been a standard operation for cholecystolithiasis. As the history of the endoscopic surgery is only about 10 years in Japan, other kinds of endoscopic surgery (e. g. laparoscopic colectomy and laparoscopic gastrectomy) are not widely used. To promote the practical use of the endoscopic surgery. residents should be educated to be experts along the guidelines of the medical societies. Telemedicine by new information technology and networks between medical center and local hospitals as well as ongoing training systems should be employed.
