Education and Training System of Laparoscopic Surgery in Gastrointestinal Surgery

　腹腔鏡下手術の効果的な教育およびトレーニング法について解説する．第1点は術式の定型化である．これには患者の体位，トロッカーの刺入部位，鉗子類などの手術器具の選定，手技のマニュアル化が必要である．第2点は，術者が開腹術でも同等の手術が施行できるだけの十分な解剖学的知識と技術を身につけていることである．第3点は，マス教育でのライヴデモンストレーションによるレクチャーコースが実際的で有用と思われる．第4点は双方向通信システムを用いた遠隔手術指導である．これにより術者は，指導者の指導を受けながら安心して手術を施行できる．しかし，遠隔手術指導には指導者不足，コスト，法的側面など解決すべき問題がある．

We describe an effective training and education system of laparoscopic surgery.The standardisation of surgi-cal procedures is the most important, including the position of a patient, trocar sites and instrumentation. Next. a surgeon has to acquire accurate and profound knowledge of surgical anatomy and skills. Thirdly, a clinical lec-ture course with live demonstration of laparoscopic surgery is useful and instructive. Lastly, an inexperienced surgeon can carry out operations under the supervision of a telementor, using a two-way communication sys-tem.
