大腸癌に対する腹腔鏡下手術のトレーニングにおけるペアリングシステムの有用性

Efficacy of the pairing system in the training of laparoscopic surgery for colorectal cancer

　当科では習熟した術者が新たな術者とペアになり一定期間連続して手術を指導し，その指導により独立した術者がさらに新たな術者とペアになり一定期間連続して手術を指導してきた．今回はこの方法の有用性を検証した．3期間（1期2000年：A,Bペア，2期2001年：B,Cペア，3期2002年：C,Dペア）の手術時間，出血量，合併症などを比較した．手術件数は1期，2期，3期の順に17,28,25件であった．右半結腸切除術，S状結腸切除術での平均出血量，術後合併症は差がなかった．全手術症例での開腹移行例や合併症も差がなかった．本トレーニングシステムは安全性を保持しつつ連続した十分な手術経験が可能という点で有用な方法と考えられた．

It has been customary practice in our department that a skilled surgeon pairs off with a new surgeon and in-structs him in laparoscopic colorectal surgery for a certain period, after that the “new” surgeon who has be-come a full-fledged surgeon pairs off with another new surgeon. We examined the efficacy of this system. The duration of surgery, the amount of bleeding, the complications, etc. , were compared among three periods: the period 1 (2000): the pair A and B; the period 2 (2001): the pair B and C; the period 3 (2002): the pair C and D.
