　開腹で行われている直腸癌の手術は開腹創から得られる頭側からの視野だけによって手術が行われており，現在普及している腹腔鏡下手術は開腹手術を模倣したものである．しかし腹腔鏡下手術における視野は，直腸を上下左右および前面方向から捕らえることが可能であるため，開腹手術に準じた頭側から尾側へ郭清を進める方法よりも，腸管の肛側を腸間膜も含めてはじめに切断し，そのあと頭側に郭清を進める方法（retrograde anterior resection）のほうが良好な視野が得られ，術中操作も容易であることに気づいた．本術式は，腹腔鏡下手術の特性を生かした術式であり，尾側への過剰な郭清と不必要な解剖学的破壊を防止できるため有用と考えたので報告する．

The rectal cancer used to be dissected from the head side to the tail-side, because the open surgery was based on the surgical field viewed only from the head-side. Laparoscopic surgery for rectum, for rectum onthe other hand, has many surgical views, the surgical fields may be viewed from the up, down, right, left, and front sides. Yet, at present, dissection for rectal cancer under laparoscopy is performed from the head-side,as in open surgery. “Retrograde anterior resection” is a new surgical method. At first, an anal side of the rectum and mesorectum are cut.
