腹腔鏡下直腸癌手術におけるリニアステイプラーによる直腸間膜の一括切離

Transection of the rectum and mesorectum with endoscopic linear staplers in laparoscopic surgery for rectal cancer

　腹腔鏡下手術では，直腸切離に際しての肛門側断端の直腸固有筋層の露出が低位になるほど操作は困難になり，直腸の損傷や固有筋膜の破壊による癌細胞の散布が懸念される．われわれはそれを避けるため以下の方法で手術を行っている．まず，直腸切離に際して直腸壁の露出は一切行わず，恥骨直上のポートから自動吻合器で仙骨に垂直になる方向で直腸および直腸間膜を一括切離する．再建は肛門から挿入した自動吻合器でdoubel stapling technigueで行う．このとき，直腸断端には腸間膜組織が付着しているが取り除いたりしない．本法を12例に施行したが合併症は認めなかった．本法は直腸癌手術に際して操作を簡便にし，根治性を高めるのに有用と考えられた．

In laparoscopic surgery, exposing the distal rectum transection by excising the blood vessels, the perirectal fat, and the mesentery becomes more difficult when the lesion is situated in the lower rectum. During this pro-cedure, rectul wall damage and cancer cell dissemination due to the destruction of fascia propria near the tumor site may occur. To avoid these risks, we considered the following methods. The rectum was not exposed at the resection site.
