1991年に腹腔鏡補助下幽門側胃切除術を開発し，これまでに58例の早期胃癌に対して行った．本術式の開発に際しては，安全性と簡便性を確立することが重要であったが，超音波凝固切開装置（LCS）の開発がこれらを可能にした．すなわち，大網切離，小網切離，右胃大網動静脈切離，左胃動静脈切離などを腹腔鏡下に出血や周囲臓器の損傷なく安全に行うことができるようになったのである．その結果，手術時間は開腹術と有意な差を認めず，出血量は有意に少なかった．術後合併症は，慢性気管支炎の既往を有する患者に生じた肺炎と慢性膵炎の既往を有する患者の癒着剥離後に生じた少量の膵液漏の2例であった．ともに保存的に治癒しており，開腹下手術と同等に安全な術式と思われた．
We have performed laparoscopy-assisted distal gastrectomy (LADG) with lymph node (D1+α) dissection for 58 patients with early gastric cancer, since 1991. In this operation, we used a laparoscopic coagulating shears (LCS) for the division of the omentum, gastroepiploic vessels, left gastric vessels, and lymph nodes (No. 7 and 1) dissection. The outcomes of LADG using LCS were satisfactory because this device causes minimal tissue injury and facilitates dissection without bleeding. Thus, the LCS enabled LADG to be safe and simple operation. We herein described the techniques and outcomes of LADG using LCS and showed the usefulness of LCS.
