胃癌に対する腹腔鏡下リンパ節郭清における超音波凝固切開装置の応用

Application of Ultrasonically Activated Instrument in Laparoscopic Lymphadenectomy for Early Gastric Cancer

　胃癌手術におけるリンパ節郭清は，その解剖学的特性のため腹腔鏡下に施行するのは困難であり，多くの施設では小切開からの直視下視野を併用している．われわれは，超音波凝固切開装置と超音波メスを併用し，腹腔鏡下視野のみで各種胃切除におけるリンパ節郭清を行っている．超音波凝固切開装置は，電気メスと異なり熱損傷を併発しにくいので安全にリンパ節郭清が行える．この手技を用いた19例のリンパ節郭清を伴った腹腔鏡下胃切除（幽門側切除10例，幽門輪温存切除2例，噴門側切除4例，胃全摘3例）を施行したが，超音波凝固切開装置使用に起因した術後出血，リンパ漏などの合併症は経験していない．

Laparoscopoy-assisted gastrectomy with lymphadenectomy has been widely performed in gastric cancer. However, the complex anatomy of the stomach poses serious technical challenges for laparoscopic lymphadenectomy for gastric cancer. We carried out, laparoscopic gastrectomy with lymphadenectomy. Using both an ultrasonically activated instrument and an ultrasonic surgical aspirator. Using the ultrasonically activated instrument, problems such as burn injuries that can occur using electrocautery, can be avoided, thus improving the safety of laparoscopic lymphadenectomy.
