クリップレスD2腹腔鏡補助下幽門側胃切除術—LigaSure AtlasTMの使用経験

Clipless D2 Laparoscopy-assisted Distal Gastrectomy : Experience with LigaSure AtlasTM

　リンパ節郭清を伴う腹腔鏡補助下胃切除術の手技を簡略化する目的で，すべての血管をクリップレスで切離した．主要血管の切離には，最近開発されたLigaSure Atlas^TMを使用し，それ以外は従来からの超音波凝固切開装置で切離した．D₂腹腔鏡補助下幽門側胃切除術の15例に用いたところ，出血量は平均148mlで，全例で左胃動脈をはじめとする全血管を確実に止血切離できた．また，術後出血などの合併症もなく，術後平均12.2日で退院した．この両装置を使用することで，血管の切離を安全かつ確実に施行でき，手技の簡略化に役立つものと考えられたので，その使用経験を報告した．

In order to simplify the technique of laparoscopy-assisted distal gastrectomy accompanied by lymph node dis-section, all blood vessels were cut without clips. For the resection we used the recently developed LigaSure Atlas^TM in addition to a conventional ultrasonically activated device, so herein report our results. This technique was used in 15 patients undergoing D₂ laparoscopy-assisted distal gastrectomy.
