ストレス潰瘍による消化管出血を予防することは死亡率や予後を改善させるため，ICU患者にとって必要不可欠な管理の1つであり，臨床医にとって日常的なプラクティスとなっている。しかし，胃酸分泌抑制薬の使用は肺炎やClostridioides difficile感染症の増加，さらには死亡率上昇との関連も懸念されている。

　本稿では，ストレス潰瘍予防を適切に行うために，現行のガイドラインや近年の研究を参照しながら，危険因子を考慮した適応の判断，予防薬の選択，投与量，投与期間について解説する。また，漫然と予防を継続しないために，頻繁な再評価と予防中止の検討が必要であることを強調したい。

Stress ulcer prophylaxis is essential in the management of critically ill patients admitted to the intensive care unit, to prevent gastrointestinal bleeding caused by stress ulceration, to reduce mortality and improve outcomes. Stress ulcer prophylaxis is already a common practice for clinicians. However, there are concerns that using stress ulcer prophylaxis may be associated with an increased frequency of nosocomial pneumonia and Clostridioides difficile infection and its relationship to mortality. We reviewed available evidence for the appropriate use of stress ulcer prophylaxis in this setting, referring to several current guidelines： indications considering risk factors, the choice of prophylactic agents used, dose, and duration. We emphasize the need for frequent re-assessment of the use of appropriate stress ulcer prophylaxis.
