Venous thromboembolism prophylaxis in the intensive care unit

深部静脈血栓症（DVT）や肺血栓塞栓症（PE）などの静脈血栓塞栓症（VTE）はICUで遭遇する頻度が高く，致死的な合併症の1つである。VTE予防には薬理学的血栓予防と機械的血栓予防があり，重症患者では血栓形成のリスクが高く，早期に予防を行うことが重要である。一方で，重症患者ではそれぞれ出血リスクも伴っているため，全例に同一の予防法を適応することはできない。そこで，血栓・出血のリスク評価から，予防法，各病態における予防の選択に関して本邦での適応もふまえて論じていきたい。

Venous thromboembolism, including deep vein thrombosis and pulmonary embolism, is one of the most frequent complications in the intensive care unit. Deep vein thrombosis prophylaxis includes pharmacological and mechanical thromboprophylaxis. In critically ill patients, the risk of thrombus formation is high and early prophylaxis is important. Patients with critical illnesses are also at an increased risk of bleeding, so prophylactic measures cannot be uniformly applied to all patients. This paper describes risk assessment for thrombosis and bleeding, prophylaxis, and the choice of prophylaxis for each condition, taking into account the indications in Japan.
