はじめに
わが国では2024年における人口に占める65歳以上の割合が29.3%に達し,今後も高齢患者の手術件数は増加し続けると予測されている1)。高齢であること自体が術後合併症や死亡率など予後不良に関連していることは周知の事実である2)。しかし,一口に高齢者といっても,個々の “脆弱性” や “回復力” には大きな差があり,年齢や米国麻酔科学会における術前全身状態分類(American Society of Anesthesiologists physical status:ASA-PS)など従来の術前リスク評価指標だけではこれらを十分にとらえることができないことをしばしば経験する。この “暦年齢と生理学的年齢の不一致” を埋める概念がフレイル(frailty)である3)。フレイルは “生理的予備能力の枯渇” という本質的な脆弱性を反映しており,フレイル患者は軽微な侵襲に対しても容易に代償不全となりうる。そのため,フレイルの患者は,そうでない患者に比して周術期予後不良であり,フレイルは周術期予後の独立した予後不良因子であることが多く報告されている4)。麻酔科医の仕事は周術期のリスクから患者の安全を守ることである。こうした “脆弱性” を抱える患者の周術期を安全に導くには,麻酔科医が中心となり,全身の生理的予備能を的確に評価し,侵襲と回復のバランスを設計する視点が不可欠である。本論文ではフレイルの総論に触れつつ,なぜ麻酔科医にフレイルのマネジメントが適しているのかを明らかにし,今後の周術期医療における新たな視点を提示する。
The rapid aging of surgical patients has made frailty a critical determinant of perioperative outcomes. Frailty, defined as reduced physiological reserve and increased vulnerability to stressors, affects 20%-30% of surgical candidates and independently predicts postoperative complications, prolonged hospitalization, and mortality. Traditional risk scores such as the ASA-PS fail to capture this biological vulnerability. The perioperative management of frail patients requires a multidimensional approach:preoperative screening with tools such as the Clinical Frailty Scale, the optimization of the patients’ nutrition and cognition, individualized intraoperative strategies, and early postoperative rehabilitation. As perioperative physicians, anesthesiologists are uniquely positioned to lead comprehensive, multidisciplinary interventions. Recognizing and addressing frailty throughout the surgical pathway is essential to improve patient outcomes and resource use in the era of aging societies.
