Pathology of connective tissue disease and vasculitis

膠原病・血管炎の臨床サイドの見地から，「病理学的検討によって何がわかるか」「何がわからないか」「ピットフォールの回避」などについて，主に「皮膚生検」「側頭動脈生検」に力点をおいて総説した。膠原病・血管炎を積極的に診断できる単一の検査は存在せず，病理所見もあくまでも臨床のコンテクストに照らして解釈すべきである。炎症性病態を評価するための検体は「ホルマリン漬け」にしてはならない。また，病理検体の採取を他科に依頼するときは，必ず付き添う。病理部には足を運び，病理医がレポートに書ききれなかった「診断のニュアンス」を聞き取るようにしたい。

This review discusses, from the clinician's viewpoint, what information can and cannot be obtained by pathological examination in patients with connective tissue diseases and vasculitis, and how the pitfalls of these examinations can be avoided. Skin biopsy and temporal artery biopsy are examples of conditions for discussion. There is no single test that can effectively rule in or rule out the diagnosis of connective tissue diseases and vasculitis. Even the pathological diagnosis is not the “final diagnosis”, which should be interpreted in the clinical context. A specimen from an “inflammatory site” must NOT be immersed in formalin. Physicians should obtain adequate specimens to get an accurate pathological diagnosis, and should have a face-to-face discussion with pathologists who can provide readings “between the lines” in reports.
