薬疹の臨床病理学的検討

I．はじめに

　現在迄にもある種の薬物たとえばクロールプロマジン，サルバルサン，サルファ剤，若干の抗生物質などが肝機能あるいは骨髄機能に対ししばしば重大な影響をおよぼすことはよく知られており，内科学的，病理学的立場より，かなり十分な検討が加えられている。しかし，あつかわれた症例の多くは皮膚に発疹がなく，またたとえ存在したとしてもその程度，種類，範囲などに対しては，ほとんど考慮されず，皮膚病変あるいは皮膚機能障害という1つの要素の介在した状態における全身的機能検査については，ほとんど知られていない。今回日本皮膚科学会第18回西日本連合地方会の主題の1つとして取り上げられた「皮膚疾患の臨床病理学的検討（外因性疾患の全身状態について）」の一環としてわれわれは薬疹のそれを担当したので，西日本各施設から提供された重症薬疹例にもとづき，主として末血像，血清蛋白像，肝機能所見について検討を試みることにした。

Although it has been well known fact that penipheral blood picture and liver function tests give the specific changes in systemic drug allergy, those changes in severe aller-gic eruption of drug are not known.

This study of 9 cases of drug eruption was performed to know how these specific patterns of laboratory results of systemic drug allergy could be modified by the severe and widesp-read skin manifestations.

1) The main change of peripheral blood picture was the nonspecific inflammatory reaction, where the specific findings for systemic drug allergy such as granulocytopenia, anemia and thrombocytopenia were masked by the drug eruption. However some cases showed the nonspecific blood picture in the early stage and the specific findings in the late stage.

2) Few cases showed the specific test pattern of altered liver function for drug allergy such as hepatitis-type or intrahepatic gall retention-type. The cases which showed definite changes of liver function tests due to the damaged skin by drug allergy could not be proved.
