タイにおけるリハビリテーション医学の現状

The present status of Rehabilitation Medicine in Thailand.

はじめに

　タイは東南アジアに位置し国土総面積は51万3,120km²であり，世界第51番目に大きい^1）．そのうち陸地面積は51万890km²，水地面積は2,230km²である．気候は熱帯性でモンスーンを特徴とする．公用語はタイ語である．国民の94.6％が仏教徒である^2）．2013年時点のタイの総人口は6,462万3,000人であり^3），障害者登録人口は137万9,103人である^4）．人口対医師数の割合は1994年に1：4,165，2004年に1：3,305であった^5）．タイのリハビリテーション医学は現在，サービスと教育の両面で過去10～30年間で著しい進歩を遂げている．リハビリテーションに対する需要の増加は，国民の教育および社会経済的状況の改善や，筋骨系および神経学的疾患など非感染性疾患の発生率の増加などが原因かもしれない．タイ経済の農業からサービス・製造業への転換は，労働災害や交通事故が増加する一因となっている．高齢者割合の増加も考慮される．60歳以上の人口は2010年に10％を超えており，2050年には人口の3分の1近くに達する見込みで，高齢者はリハビリテーション医学を必要とする大集団となるであろう．

　リハビリテーション医学は，人々の日常生活に徐々に浸透している．今や人々は健康やQOLにより一層の関心を払うようになっている．以前は，リハビリテーションを必要とした人のほとんどはほぼ麻痺患者と身体障害者であったが，もはや状況は一変した．近年リハビリテーション医学の需要は増加しているが，リハビリテーション医療サービスは，依然として地方よりもバンコクとその周辺の都市部に集中している．タイの経済発展は，貧富の格差を縮小するどころかより大きな収入格差を生み出していると批判されている．貧困層や地方の住民は今も良質のリハビリテーション・サービスを利用できていない．この問題に対処するため，タイの医療システムに関する国家計画戦略の1つに，有資格の医療スタッフの研修をさらに強化することが挙げられている．しかし，医療スタッフの偏在はタイにおける主要課題の1つである．

Abstract：This paper aims to present the current status of the Rehabilitation Medicine in Thailand including the education system of physiatrists, physical therapists, occupational therapists, orthotists and prosthetists as well as speech pathologists. The history of Rehabilitation Medicine in Thailand is also prescribed. The manuscript also mentioned about the imparity of Thai socioeconomic status which interfering the progression of rehabilitation field in Thailand. The poor distribution of health personal, health education and health service system are still be the big problems. The future prospect of Rehabilitation Medicine in Thailand is the internationalization and collaborating research for the development of Rehabilitation Medicine for global requirements.
