I．はじめに
めまいは，主に，内耳，小脳，脳幹の障害により生じると考えられ，大脳は，めまい発生にあまり関与しないかのように考えられてきた。大脳障害において，意識障害，失語症，片麻痺，その他生命の危険を伴うことが多いため，めまいという訴えは，気づかれないことが多いのではなかろうか？しかし，てんかんの前兆としてのめまい，大脳の出血の危険信号としてのめまいは，古くから知られている。
今回，大脳障害により生じたと考えられるめまいを主訴として来院した症例を経験したので報告すると共に，そのめまい発生の機序を考えてみる。
Six patients of cerebral lesions who complained of vertigo, 2 tumor cases, 3 vascular lesions cases, 1 epilepsy case are reported. Five cases had lesions near the Sylvian fissure and one showed the slow and spike waves of EEG at the temporoparietal lobe.
The area 2v of rhesus monkeys has been regarded as the vestibular cortex. Therefore, the lesions around this area may cause vertigo or vertiginous sensation.
The lower part of the parietal lobe, area 40 and 39 are very important for optokinetic nystagmus or visual fixation.
