症例にみる内耳部分障害

CLINICAL ASPECTS OF PARTIAL INNER EAR LE-SIONS

　I．はじめに

　内耳は，左右両側にあり，各々に，蝸牛（1個），半規管（3個），耳石器（2個）があり，その機能は，聴覚と平衡覚に二大別される。

　難聴，耳鳴，めまいを訴える症例の中には，これら6個の感覚装置がいろいろの組合せで，また単独に障害されていると診断される症例に遭遇する。すなわち，これら6個の感覚装置がすべて障害されている場合，蝸牛のみの障害の場合，内耳前庭部分，またその一部のみの障害と診断される症例など，その組合わせは多数あり，このためその病像はきわめて多様である。

　The inner ear includes the cochlea, semicircular canals and the otolithic organs. In otological cases all of these organs may be affected simultaneouslyor partially affected with various combinations. Recent advances in the study of the inner ear have made it possible, in most cases, to diagnose partial lesions.

　Cases of cochlea lesions may be combined with disorders of the vestibular organs or visa versa. Itis important that, in the inner ear lesion cases, both hearing and vestibular tests are performed. Because of the fact that changes may occur rapidly in the inner ear lesions, the patient should be repeatedly and carefully examined during the course of their hospitalization.
