鼻腔血管腫の手術的療法

Surgical Treatment of Intranasal Hemangioma

　I．はじめに

　血管腫の多くは自然退縮性を持つと言われている。しかし，鼻腔血管腫の場合は，鼻の機能障害，繰り返す大量の鼻出血，それによる気道閉塞などをきたす恐れがあり，血管腫が自然に退縮するのを待つ時間的余裕はないことが多い。したがって，可及的早則に血管腫を摘出しなければならないが，その成否は出血に対する予防策をいかにうまく行うかにかかっている。

　筆者らは3例の鼻腔血管腫を経験し，あらかじめ腫瘍の栄養血管への血流を阻止することにより，大量の出血をみることなく安全に摘出手術を行いえたので報告し，参考に供したい。

　In the reported three cases with extensive intranasal hemangiomas, transantral microsurgical clippings of the maxillary artery and its branchings were performed. In one case, ligation of the right external carotid artery, and in the others, clipping of the contralateral maxillary artery, ligation of the external carotid artery and electrocoagulation of the ethmoid artery were supplemented. In large intranasal hemangioma, angiography using subtraction technique is valuable to find multiple feeding arteries which should be blocked to prevent excessive bleeding during operation.
