視力障害をきたした巨大な篩骨蜂窩，蝶形骨洞ピオツェーレの1症例

A LARGE PYOCELE OF THE SPHENOID AND ETIIMOID SINUS PRESENTING REPETITIVE VISUAL DIFFICULTIES. REPORT OF A CASE

　I．緒言

　眼症状を伴った副鼻腔疾患については，Onodi^23,24）による後部副鼻腔と視神経管の詳細な解剖学的研究の報告以来数多くの報告があるが，蝶形骨洞や後部篩骨蜂窩のムコ・ピオツェーレ症例は，文献上比較的稀であるといわれている。眼症状，頭痛などを主症状とし，鼻症状を欠くことが多いため，眼科や脳神経外科を介して耳鼻科を受診する場合もあり，頭蓋内疾患と誤られ開頭術を受けた後に初めて後部副鼻腔嚢腫と診断されることも少なくない^4,31）。

　最近筆者らは，眼症状を伴った右篩骨蜂窩と両側蝶形骨洞とが一つに融合した巨大なピオツェーレの1症例を経験する機会を得たので，文献的考察を加え報告する。

　A man aged 40 with a large sphenoid and ethmoid sinus pyocele developed headaches, vomiting, and visual disturbance. Radiological studies showed the destructive finding of the sphenoid sinus and opacification of the ethmoid and sphenoid sinuses. The lesion produced displacement and stenosis of the internal carotid artery on arteriogram. The outline of the lesion was clearly showed in the preoperative CT-scan.

　The symptoms were perfectly improved after surgery by extranasal approach to the lesion.
