蝶形骨洞と篩骨洞に生じた腺様嚢胞癌の1例

A Case Report of Adenoid Cystic Carcinoma in the Sphenoid and Ethmoidal Sinuses

　はじめに

　腺様嚢胞癌（adenoid cystic carcinoma）は，耳下腺，顎下腺，舌下腺などの大唾液腺や口腔，鼻咽頭の小唾液腺，気管，気管支，涙腺や乳腺などの腺組織に発生する腫瘍である^1）。また，蝶形骨洞を占拠部位とする悪性腫瘍は多くなく^2〜8），そのうち腺様嚢胞癌は稀^9〜11）とされている。

　今回われわれは，蝶形骨洞と篩骨洞に生じた腺様嚢胞癌の1例を経験したので，文献的考察を加えて報告する。

A 49-year-old man had the chief complaint of nasalobstruction.

Surgical removal of a tumor was carried out. Then, local recurrence was recognized and tumor invasion was thought from the sphenoid sinus, and cavernous sinus to the middle fossa. The reduction of tumor was not suc-cessful by chemotherapy, radiotherapy treatment. The patient developed diffuse metastasis to the bilateral lung field, but he survived to present time.
