慢性副鼻腔炎再手術例の検討

AN EVALUATION OF RE-OPERATION IN CHRONiC SINUSITIS

Ⅰ．緒言

　慢性副鼻腔炎の手術療法は，過去長い間手術成績の向上を目指して，多くの業績が積重ねられてきた。すなわち根治手術は，Caldwell-Luc氏法を基礎とし，日本においては，西端の唱えたPan-sinusectomy^1）と発展してきた。現在の段階においては，少なくとも手術法に関する限りは，ほぼ完壁な域まで到達したと考えられる。種々の手術のうち，上顎洞，篩滑洞に対しては，西端の保存的多洞手術Conservative Polysinusectomyが，日常もつとも多く行なわれているが，その成績について，西端^2）は，後療法を巧く行なつて70〜80％，手術が巧く行なわれず，また後療法が不十分となると，60％位であると述べた。

　このように手術成績が，いまだに満足できない事実は，臨床家にとつて非常に大きな問題である。多くの先賢により，この原因が検討されてきたが，その主なものを要約すると，

　1）病的粘膜（蜂窩，小腔）の遺残がある場合。

　2）副鼻腔病変は，ほぼ治癒していても，鼻腔病変に考慮が払われなかった場合。

　3）特定洞のみ手術され，他洞は手術されなかつた場合。

　4）術後創の変貌，再生や感染が起こった場合。

　5）後療法による場合。

　を挙げることができる。

　An evaluation on 12 cases (22 sides) of chronic sinusitis that required re-operation was made. The principal causc by which recurrence of sinusitis was brought on was considered as the ineffectual initial operation performed intranasally which failed to effect a complete removal of the affected mucous membrane resulting in its adhesions and vacuolizations. For this reason, the authors emphasize, that it is highly essential that, in sinus operations, the initial operation be performed with a great care and proper correction of nasal, deformity.
