上顎洞篩骨洞脂肪移植充填手術の検討

Ⅰ．緒言

　慢性副鼻腔炎に対する種々の手術的療法のうち，術後創腔を充填する手術法は，主として前頭洞に対して行なわれてきた。前頭洞充填手術については，Bergara^1），Tato^2），Goodale^3）らが主に自家脂肪組織を充填し，佐々木（寛）^4），高橋^5）らは自家腸骨を使用し，いずれも良好な成績を得たことを報告した。

　最近上顎洞に対する充填手術についても報告されるようになり，飯田ら^6）（1963）が自家脂肪・腸骨を充填した5症例を報告し，Kiel boneを充填した岩沢ら^7）（1965）の報告もある。McNeill^8）（1967）は慢性上顎洞炎10症例に対し上顎洞前壁を骨形成的に処理して骨弁とし，自家脂肪を充填して，良好な遠隔成績を得たことを報告した。

　Occlusion operation upon the maxillary and ethmoid sinuses in 8 cases were performed by using autotansplant of fatty tissues as material of padding the sinus cavity. The fatty tissues transplanted in this way showed a good take unef fected by the surrounding inflammatory changes occuring in the mucous membrane and bony tissues but, they were gradually replaced by fibroconnective tissues.
