耳鼻咽喉科領域手術に対するオスモカインの局所麻酔効果について

　まえがき

　従来より耳鼻咽喉科領域の手術に際して，塩酸プロカイン（以下塩プロと略）は最も多く利用されている局所麻酔剤であるが，時には之のみでは十分に効果の挙がらない場合も経験される処である。

　オスモカインは塩プロに拡散因子であるヒアルロニダーゼ（スプラーゼ）を配して，その拡散作用により塩プロの局所麻酔作用の増強をはかつた薬剤である。従つて之を局所に注射すれば麻酔作用を迅速，的確，且つ広範にわたつて発現させる事が出来る。この方法は欧米においては勿論，本邦においても盛んに研究され，多くの報告がなされており，耳鼻科領域でも大沢，高須氏等の報告がみられる。而し従来のヒアルロニダーゼは水溶液中における安定性に対して難点があり，之を溶解すると比較的早くその効力を減じ，又使用が煩雑に考えられて充分に実用化されていなかつた。最近持田製薬において，この点を考慮し，溶液中での安定度の高いオスモカイン溶解用スプラーゼの製造に成功した結果，使用法が簡便にして，実用上十分に利用し得る局所麻酔剤オスモカイン注射液を完成した。吾々はその提供をうけたので，之を耳鼻科領域の手術に使用してみて，利点の多い局麻剤であることを確認したので，本剤を紹介し，その使用経験を報告する。

Kaneko and Takami state that the use of osmocain which is procain in combination with hyaluronidase is highly advantageous in the surgery of ear, nose and throat. The agent is characterized by high degree of spreading effect, producing no swelling at the injection site it is suitably employed in surgical pro-cess involving skin surface. By the same token and being easily managed for local ane-sthesia for large areas of tissues the use of this agent is the most suitable for surgery in the field of otolaryngology. Moreover, in cases of reoperations where adhesions are fret this agent may be used to a great advantage.
