耳鼻咽喉科領域における局所麻酔の偶発事故とその対策—特にアドレナリン過敏症について

　緒言

　耳鼻咽喉科領域に於ける手術は，その解剖学的特殊性や手術時体位等の関係から，他科の手術に比較して局所麻酔による偶発事故が可成り多いと云われている。1948年から1958年までの10年間の東京都監察医務院の統計によれば検案44336例中，診療事故死は120例であり，このうち局所麻酔による偶発事故死は42例，塩酸プロカイン使用例は23例であるが，産婦人科3例，外科2例，歯科及び内科各1例に対し特に耳鼻咽喉科領域が16例（54.8％）を占めているのは遺憾である。さらに詳しく鈴木の統計によると，昭和7年から32年までの約25年間の我科領域の偶発事故42例中，扁桃剔出術及びその麻酔によるものが22例（52.4％）の多きを数え，中耳手術7例，気管支鏡操作3例，扁桃周囲膿瘍試験穿刺，口蓋裂手術，喉頭手術各2例，その他4例となつている。即ち局麻の偶発事故は我科領域が約半数を占め，さらにその約半数が扁剔術の際に起つていることが判る。そして従来これらの多くは単純に塩酸プロカインによる急性中毒とみなされてきた。元来，我科の手術は術野がきわめて狭くかつ実質性出血が多くて止血しにくい為，一旦出血すると視野が全く塞がれ手術操作は困難を極めるのである。従つて一般に麻酔効果の増強，時間の延長と中毒作用軽減の目的で局麻剤に血管収縮剤を添加するのが常であるが，我科では特に他科に比べて術野の止血の目的のためこの血管収縮剤を大量添加する傾向があつた。成書にも局麻剤に対する血管収縮剤の稀釈濃度は5乃至10万倍とあり，或いはさらに大量を添加する人もある。近年，我々は扁剔術の際，この局麻添加剤（アドレナリン）に対する過敏症の為ではないかと思われる偶発事故5例に遭遇したので，改めてこれらの成立機序及びその予防対策について各方面から検討し，ここに報告する次第である。

It is said that the operations in the field of O. R. L. have more local anaesthesical accide-nts than those of others, because of local anatomical reason and special position as ope-ration. Most of them have been seemed as simple acute procain hydlochloride toxicosis, but we happened to see five accidents in the tonsillectomy, the reason of which were seem-ed as abnormal reaction (i. e. hyperegy) to adrenalin added to local anaesthesics. Our genuine research found that mechanism of the accidents consisted of external and inter-nal facters. The ones are anaethesics and how to give them, the others are patients' general states and specifities of the patients suffered from tonsillitis chronica, above all focally inffected (unstable vegetable nerve, latent heartinsufficence, and so on. ) So we think we can from today provide against these accidents, if we understand these causes and catch patient's general states be-fore operation, and prepare suitable manage-ments.
