頸椎後縦靱帯骨化症に対する前方手術法の検討

Anterior Decompression for Ossification of the Posterior Longitudinal Ligament of the Cervical Spine

　頸椎後縦靱帯骨化症の病理形態学的検討や剖検例の所見をもとにして，これまでに後方除圧術が多くなされて来たが，最も合理的と考えられる前方除圧術について検討した．本法は椎体前方削開により骨化巣を摘出し，後，椎体削開溝に骨移植を行うもので，この操作により，後方へ圧迫されていた頸髄を前方の正常位にもどし，かつ，骨移植を行うことにより頸椎の安定性をもたらすものである．原則として，骨化症により頸髄症状が急速に発現している場合は本法の適応となる．除圧が前方からなされるためにC₂以上，Th₁以下に骨化巣が拡がっている場合は本法の適応ではない．手術時に注意すべきことは，骨化巣と硬膜との癒着が広範囲な場合で，椎体削開後に骨化巣が硬膜と共によく浮上して来るならば，この癒着部の骨化巣を完全に摘出する必要はない．本法を8症例に対して施行した．これら全例に術前，痙性四肢麻痺，歩行困難，知覚障害が認められ，3例に排尿障害が見られた．これら症例では，前方除圧術の後，痙性四肢麻痺と知覚障害の著しい改善と歩行の正常化，および排尿障害の消失または改善が見られたが，1例においてC₅の高位で右側方に存在せる骨化巣を限局性に取り残し，これが浮上せる頸髄の一部を圧迫したために惹起されたと思われる右上肢の運動障害が術後残存している．本法を成功に導くには頸髄に対する完全な除圧と愛護的操作が望まれる．

Based on detailed pathological and anatomical studies on ossification of the posterior longitudinal ligament of the cervical spine^1,4,5,6) and reliable reports on autopsy cases^8,9), various methods of surgical treatment for this pathological condition have been devised and recommended for use.

These operative procedures may be divided into two types-procedures by posterior approach^2,3) and those by anterior approach¹²⁾. In the anterior decompression the ossified area of the posterior longitudinal ligament is excised first and then the resulting defect is filled up with a hone graft.
