I．はじめに―Multiple System Atrophyとはその問題点

　multiple system atrophy（多系統萎縮症）の病像解析というテーマを与えられたが，multiple system atrophyとはいかなるものかについて神経内科医や神経病理学者の間で人による意見の違いがみられる。そもそもmultiple system atrophyという言葉はGraham and Oppenheimer^4）（1969），Bannister and Oppenheimer^2）（1972）の報告に出てくるものであるが，それらの論文はいずれもShy-Drager症候群（SDS）の剖検例を扱ったもので，その病変がmultiple sitesにあるところから問題が発している。彼らの指摘をまつまでもなくオリーブ橋小脳萎縮症（olivo-ponto-cerebellar atrophy，OPCA），線条体黒質変性症（striatonigral degencration，SND）もmultiple sitesに萎縮を来たす疾患である。このような疾患では小脳系および錐体外路系，ならびに自律神経系が侵され，まさしくmultiple systemに萎縮を生ずるが，その侵され方は一様でなく，症例により病変の程度が異なる。

　On the basis of neuropathological studies, Graham et al. (1969) and Branister et al. (1972) described socalled multiple system atrophy, in which Shy-Drager syndrome (SDS), olivo-ponto-cerebellar atrophy (OPCA) and striatonigral degeneration (SND), as well as intermediate or transitional cases of them, were included. From a clinical, i.e., scmiological and pathophysiological standpoint, we have been investigating clinical features of the cases of OPCA, SND, SDS and dentatorubropallidoluysian atrophy (DRPLA) which all have combined degeneration of the cerebellar and extrapyramidal systems.
