被殻，黒質—オリーブ核，橋，小脳—脊髄に変性のみられたパーキンソン症候群の1例—Multiple System Atrophyとの関連において

A case of Parkinson syndrome with putamino-nigro-olivo-ponto-cerebello-spinal degeneration: With special reference to multiple system atrophy

I．はじめに

　臨床的にパーキンソン症候群，起立性低血圧などの自律神経症状を伴ない，剖検により病変主坐が脊髄—オリーブ核，橋，小脳および被殻，黒質にみられる1例を経験した。それぞれの病変は従来，striatonigral degeneration（SND）^1〜3），olivo-ponto-cerebellar atrophy（OP-CA）やShy ＆ Drager症候群^4）といわれるものの脊髄病変に一致する。これらの疾患の臨床症状，病理所見の中には従来の報告例でも重複する場合があり，またこれらが臨床概念なのか，それとも病理所見に基づく診断名なのかは，必らずしもはっきりしていない。一方で，multiple system atrophy^5,8）という考え方もかなり敷衍化してきている。

　本症例はこのような点に問題提起をする1例と思われるので，ここに報告し御検討をあおぎたい。また被殻にみられた顆粒の性状について二，三の知見を得たのでここに追加する。

　1) The patient was a 66-year-old male with Parkinson' syndrome, orthostatic hypotension and dysuria. Cerebellar signs were negative. He did not respond to L-DOPA therapy and die after 6 years of illness.

　2) Pathologically degenerative changes were observed bilaterally in putamen, substantia nigra, inferior olivary nucleus, pons. cerebellum, and intermediolateral nuclei of spinal cord. Neither Levy's bodies nor neurofibrillary tangles were noticed.
