　症例は22歳，男性。10歳のとき，会陰部にボルトを挿入されたが無症状のため放置していた。会陰部の有痛性腫脹および尿閉を主訴に，当科を紹介され受診した。骨盤部単純撮影にて，会陰部に輝度の高い異常陰影を認めた。以上より，金属ボルトに起因する壊死性筋膜炎を疑い，同日に異物摘出術および切開排膿術を施行した。ボルトが2本，尿道海綿体に接し並行に埋没し，異物摘出後，尿道に直径2cm弱の穿孔を認めた。術後1年間，特に後遺症は認めていない。

A 22-year-old male presented with complaints of scrotalswelling and urinary retention. At 10-year-old, he had an episode of some metal bolt forcedly inserted in perinealregion. A plain firm showed two pieces of homogenousmaterials with high brightness. Under the diagnosis ofnecrotizing fascitis by metal bolts, extraction of foreignbody and drainage surgery were performed. Two metalbolts buried in parallel with corpus spongiosum penis andurethral perforation in 2cm diameter was seen at the anteriorurethra. After 1 year, he has no complications.
