外傷性陰茎転位

TRAUMATIC DISLOCATION OF THE PENIS

　本邦第1例目の外傷性陰茎転位を経験したので報告した．29歳，男性．オートバイの交通外傷にて来院．本来陰茎のある部位に陰茎を認めず，同部に陥凹をみるのみだった．手術を施行，用手的に陰茎を整復することに成功した．陰茎は包皮のほぼ全周にわたる挫創のみを認め，挫滅された包皮を切除後縫合した．尿道損傷および陰茎白膜損傷を認めなかった．左右の精巣は，それぞれ恥骨後面，右外鼠径輪に転位しており，両側精巣固定術を同時に行った．手術後経過良好である．

We experienced the first case of traumatic dislocation of the penis in Japan. The patient was a 29 yearsold male and admitted to our hospital after motorcycle accident. His penis retracted under abdominal skinleaving a small hole just below the pubic bone. We manually pulled out his penis and put it in placesuccessfully. The foreskin of the penis was cut apart all around under the glans. We cleaned the edge of theforeskin and sutured both edges of the wound. No injury was observed in his urethra and tunica albuginea.His right testicle was found in his right inguinal canal and his left one was located just behind the pubic bone.
