腎切石術の後出血に対し動脈塞栓術が有効であつた1例

POSTOPERATIVE RENAL HEMORRHAGE SUCCESSFULLY TREATED BY TRANSCATHETER EMBOLIZATION

はじめに

　腎切石術ではときおり術後に尿路への再出血を認めることがあるが，輸血を必要とするような後出血は稀である^1）。しかし，後出血は腎切石術の重大な合併症のひとつであり，外科的治療を必要とすることがある。外科的治療としては，再手術の困難さから主に腎摘出術が施行されてきている^2）。われわれは重篤な後出血に対して外科的治療に代りtranscatheter embolizationを試みたのでここに報告する。

A case of postoperative renal hemorrhage was successfully treated by selective transcatheter embolization with pieces of Spongel. Although a renal scintigram 16 days after embolization showed an irregular low activity area in the upper part of the right kidney, an excretory urogram showed a well functioning right kidney. No serious complication was noted after embolization, and three months later the patient remained normotensive and free of hematuria.

This method is an effective alternative to an operation in cases of renal hemorrhage not responsive to conservative management.
