はじめに

　腎結石に対する腎保存手術の意義についてはあらためてのべる必要はないであろう。

　腎盂切石術が可能であれば，腎機能に与える影響がもつともすくない点からみても好ましい手術であるが，すべての結石症例に適応があるとはかぎらない。その点で，現在まで腎切石術や腎部分切除術が行なわれてきているのが実状であり，その際には多くの症例で腎茎部遮断による血流停止が要求され，出血減少と視野確保という利点はあるものの腎実質に与える影響を常に考慮に入れる必要がある。

The aim of renal hypothermia is to reduce renal cellular metabolic activity on a temporary basis so that the cells of the renal parenchyma will tolerate a period of total ischemia.

Among the various methods of renal cooling, the authors have employed the technique described by Gibbon.

The kidney is surgically exposed and the renal artery dissected free from the hilar structures. Separate tapes are used to suspend the kidney from either pole and isolate the dissected renal artery for easy identi-fication. Systemic mannitol is our main protective agent against renal ischemic injury.
