　はじめに

　頭頸部癌では，根治術に伴う嚥下・構音などの生理的機能の障害や喪失ならびに顔面の醜形が問題となることが多い。その際，患者にとってのこのような問題点を少しでも軽減させることが，癌の根治とともにわれわれ臨床医に与えられた命題である。そこで当科では，舌半側以上の切除術を行った症例に対しては，遊離腹直筋皮弁や大胸筋皮弁を用いての再建術を施行している。しかし，一般に舌全摘出術を行った揚合，術後の誤嚥の問題から喉頭を温存できない症例が多い。

　今回われわれは，中咽頭癌再発症例（rT4NOMO）において，喉頭を温存し舌全摘出術および遊離腹直筋皮弁による再建術を施行し，術後の嚥下・構音機能を比較的良好に保つことができた症例を経験したので報告する。

The most severe postoperative problems after total glossectomy are speech and swallowing dis-orders. In a 51-year-old man with recurrent meso-pharynx cancer, MRI revealed a mass in the right side of the base of tongue. The mass extended to the right side of the oral floor. Pathological examina-tion of the mass showed recurrence of squamous cell carcinoma. The patient underwent total glos-sectomy and the tongue was reconstructed with a recto-abdominal myocutaneous free flap by means of plastic surgery techniques. The patient's speech and swallowing ability became acceptable after surgery.
