内耳障害が先行した再発性多発軟骨炎の1症例

A Case of Bilateral Inner Ear Disturbance with Delayed Onset of Relapsing Polychondritis（RP）

I．はじめに

　再発性多発性軟骨炎（relapsing polychondritis：以下RPと略）は，全身の軟骨組織および軟骨と共通した成分を含む組織が系統的に侵される原因不明の疾患である。軟骨組織にはコラーゲンが多く含まれているが，RPでは抗コラーゲン抗体が検出される例があること^1,2），炎症部位に抗体，補体の沈着がみられること^3），膠原病を合併する例があること^4），一般に副腎皮質ホルモン剤が有用であることなどから，自己免疫性疾患という考えが支持されている^5）。

　再発性多発軟骨炎は，耳介軟骨，鼻軟骨，気道軟骨の炎症と変形，眼症状などを代表とする多彩な臨床症状を呈し，再発，寛解を繰り返し進行する。また，それぞれの症状発現時期が不定なので，本症の存在が念頭になければ診断に苦慮することも多い。

　われわれは，初発症状としては比較的少ない内耳障害が先行した再発性多発軟骨炎の1症例を経験したので報告する。

　A 35-year-old man was complaining of vertigo and sudden onset of sensorineural hearing loss in his left ear. An initial diagnosis of sudden deafness was made and treatment was initiated with predonisolone with a mildly effective result. One month later,he developed sensorineural hearing loss in his right ear,and six month later,bilateral polychondritis began to manifest in both auricles. The diagnosis of RP was established. Over the follow-up period of two years,his vertigo disappeared but the bilateral hearing loss has not shown improvement.
